About Us

The nurse practitioner-led primary care family practice at The Ohio State University Wexner Medical Center East Hospital provides an interprofessional team approach to integrated physical and mental healthcare for people of all ages.

Total Health and Wellness became a Federally Qualified Health Center (FQHC) in 2019. The FQHC designation recognizes community-based healthcare providers who deliver primary care services in underserved populations. FQHCs are supported by federal HRSA Health Center Program grants, which come through the U.S. Department of Health and Human Services, aimed at expanding access to affordable, quality primary care to vulnerable areas. In 2024, the expanded FQHC presence was renamed to The Ohio State University Total Health Care Center, representing the greater breadth of holistic, interprofessional, team-based care that the practice offered the community.

Congratulations to Sarah Kincaid

Sarah Kincaid has been selected for the American Association of Nurse Practitioners (AANP) prestigious inaugural cohort of the Loretta Ford Visionary Leadership Program. This distinguished program recognizes emerging nurse practitioner leaders who demonstrate exceptional potential to advance the profession through innovation, advocacy and transformative leadership. The members of the inaugural cohort were honored during a celebratory pinning ceremony on June 24, 2026, at the AANP National Conference in Las Vegas, Nevada. This remarkable achievement reflects Sarah’s dedication to excellence and leadership, and her commitment to advancing healthcare and the nurse practitioner profession.

The Ohio State University Total Health Care Center is recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance (PCMH). PCMH is a model of care that puts patients at the forefront of care. It is designed to build better relationships between patients and their clinical care teams.
